- Name of person placing the order or a contact person
- Phone number of dental office or school
- Shipping address

If you are a dealer who does not have an account with AllPro, please contact us by e-mail at AllPro.Info@teamtech.com to arrange for a credit application and other information that may be required for you to become an AllPro dealer.
